Bloomberg’s recent BusinessWeek article on the looming eldercare crisis in China reinforces many of the points industry watchers already know:  the demographic need is immense and the list of interested people who want to get into the market grows longer every day.  Established eldercare operators of almost every stripe and flavor from around the developed world are eager to find a model that will work in China.  Many pivotal questions need to be answered about what the right model for China is, not least of which is the role retirement communities versus in–home care will play in meeting the country’s needs. While in–home care is in many ways complementary to formal retirement community living and care–giving, in–home care may also ultimately be a more flexible and cost–effective mechanism for addressing the overwhelming number of Chinese people who need care, many of whom may never have access to the financial critical mass necessary to move into caregiving facilities.  Equally of interest, the infrastructure necessary to successfully conceptualize, train and implement in–home eldercare may very well lay the groundwork for the same sort of home healthcare business model which is still evolving in the United States but has application to the general populace, to gather steam in China. One of the world’s leading in–home eldercare providers in Omaha, Nebraska based Right at Home.  Blake Martin, their Vice President of International Operations, very generously set aside an hour to speak with me last week about his company’s experience in China.  The services Right at Home provides include safety supervision, companionship, bathing, and hygiene care, meal preparation, and housekeeping from a few hours a day to 24 hours a day. Right at Home has international franchises in the United Kingdom, Brazil, and now China as well; but, as Blake acknowledged, China has presented them with some unique challenges that are being addressed as they continue expanding in the country.  At the simplest of levels, part of what Right at Home faces is what almost any business in pursuit of the Chinese market for any product quickly comes to terms with:  the size of the country makes your marketing team’s mouth water, but getting focused and picking the right strategy is a real operational challenge.  As Blake put it, “The sheer numbers, the lack of available facility-based care or any sort of residential care simply isn’t there, and the need is on a much greater scale than what we have seen in other developed nations.” Despite all the differences, it helps Blake to see that there are actually many similarities between what motivates a North American family or independent senior to choose in–home care and what motivates a Chinese family when they make similar choices.  According to Blake, just like in the United States, “people want to stay in an environment they are familiar with.  They are going to face not only significant emotional strain but also physiological stress from a movie like this.  For them, the convenience and familiarity of being at home is a strong motivator.” Blake was quick to point out that in–home care is in many ways complementary both to facility–based solutions, and also to families who may not have the financial bandwidth to either pay for relocating their parents to a stand–alone community, or for full–time in–home care.  As Blake put it, “many Chinese are living with or close to someone who is providing care.  The services that a home care agency like ours provides are simply supplements to what the family or their loved ones are capable of providing.  We are a respite.  The family is the primary caregiver, we simply provide a break and relief.” This remains one of, if not the most important, factors in determining which solution proves more adaptable in the Chinese market:  dedicated communities versus in–home care.  Both may well prove to be successful but in terms of the ability to broaden beyond the high–wealth niche existing planned community developers are targeting with their initial expansions, into the middle and even lower-middle-class Chinese families who need help but cannot afford a retirement home, in–home care like what Right at Home is offering may prove to be the more economical solution.  If so, in–home care could well find itself on a growth curve independent of, and asymmetrical to, the growth of residential eldercare communities.

A recent article in the International Journal of Urology caught my attention. Titled “Managing the Aging Man in Asia: a Review”, it emphasized the growing need for properly designed healthcare strategies reflecting the needs of Asia’s aging male population. As the article notes, “The majority of the world’s aging population is in Asia and this will continue in the future.” This is obviously one of the many reasons I am very bullish on the investment and business opportunity for senior healthcare in Asia; China specifically, but more broadly across the APAC region as well. The article makes the latter point forcefully when the authors write, “the rate of aging in Asia countries (of which the majority are developing nations) is much faster than developed nations. An increase of more than 250% in the number of people older than 65 years old is expected in many Asia countries, such as Singapore, India, Malaysia, Bangladesh, and the Philippines.” It was a temptation while reading the article to think that the issues in Asia will be no different than those faced in the developed West, at least in terms of the types of disease and the stereotypical male reluctance to get healthcare; however, What the article makes clear is that the needs of Asia’s “aging man” are going to be unique. ” These high smoking rates mean that absent large social programs designed to discourage smoking, head, neck, and lung cancers are likely to be common across Asia’s male population. The authors also note that diabetes “has been increasing throughout Asia and the speed of increase is much faster than it is in Western countries.” As Lua has written about here, the changing diet of Chinese, in particular, is setting in motion a number of ailments, diabetes most commonly. In this article, the authors note, “The westernization of Asia dietary intake as well as the sedentary lifestyle associated with a more developed world is a contributing factor.” Beyond cancer and diabetes, the authors note that male–specific disorders like erectile dysfunction are becoming more of a problem in Asia, as evidenced by studies of hypogonadism, diminished fertility rates, and lower urinary tract problems. The diagnostic and procedural techniques for treating many of these maladies are well known, but what the authors focus on is what they call “mean’s health care utilization and health–seeking behavior.” Here, they note that the traditional male stereotype in Asian cultures looks down on men that cannot bear their disease with stoicism. This inevitably leads to higher costs later on when preventative care could have pre–emptively dealt with a problem. It is the design and delivery of healthcare services for men’s needs – not only physical but cultural – that is where the most attention is needed, as the authors well know. They suggest that health care must be “tailored to men’s needs.” As they see it, this means getting men to view healthcare as a partnership with their doctor, ensuring privacy, making it easy to discuss private matters especially, and organizing community–level programs that engage men in a context they are comfortable responding to. How this is done in the various countries this article’s data covers is going to be very difficult, but will be essential if Asia’s aging man is to receive the care he needs.

For my credit cards, I generally prefer cashback rewards – as much as I travel, cash is just as easy to use towards my travel, plus it’s a lot less hassle. However, I made an exception to this rule for the US Airways Dividend Miles Premier World Mastercard, as it offers some unique perks beyond just earning miles. The real decision-maker for me was that I realized that I travel so frequently, that it was time to try to consolidate all of my airline travel to one airline, and start to reap the rewards of elite status such as free first-class upgrades.  I live in Phoenix which is a hub for US Airways, so they have lots of flight options, and I’ve generally had pretty decent experiences flying with them, as well as a decent amount of miles racked up, so I made the decision to commit to US Airways for 2011. I’m not quite at the first tier of elite status yet, but I don’t envision it taking that long (25,000 miles to hit Silver status). My favorite perk of this credit card is Zone 2 boarding group. The only people that board before you are First Class and Zone 1 – and Zone 1 is just anyone with elite status (generally they’re in First Class anyway). Now that every airline charges to check bags, I always bring my trust carry–on bag which is about as large of a carry–on as they’ll allow…as such, it’s important to get on the plane early, so there is plenty of overhead space to stash my bag. Zone 2 makes it really easy to find space for my bag, get my seat, and relax while everyone else boards. The other cool perks include a free companion pass per year, free flights for only 20,000 miles (versus 25,000 miles without the card), and usually a pretty hefty mileage bonus after your first purchase (I think I got 20,000 miles after my first purchase), so you pretty much get a free flight upon signing up. After using the card for a few months, I’m happy with it. Their online banking/account management is a little old school compared to the interface of someone like AMEX, but hey, it gets the job done, and I can pay my bill online, which is all I really care about. My initial credit limit was pretty decent, nothing amazingly high but nothing insulting low like the credit limit traps that Capital One loves to play. The only downside is the annual fee of $89. If you’re not going to be flying a lot, and you’re not planning on sticking to US Airways as your primary airliner, then I wouldn’t recommend this card. But if you’re going to be flying a decent amount on US Airways, it’s totally worth it. Even for just 5 round trips per year, that’s 10 headaches you’re saving yourself with the early boarding, and the free miles for signing up are worth the $89 themselves.  Covering yourself with LifeLock before activating your credit card will ensure no one else is capable of stealing or opening a card in your name which could cause epic headaches. So there you have it – I can honestly say I’m happy with it, which is more than I can say with a lot of other credit cards I’ve had, and my frequent flyer miles have skyrocketed since I signed up – at the time of writing, I have a little over 65,000 miles at my disposal, and I’m 10% of the way to Silver status (only 1 month into the year…).

I recently flew on Business Select on my last Southwest flight – and although it was only a $35 upgrade, I’m not really convinced it was worth the extra money. I enjoy Southwest as an airline and fly them a lot, but I don’t think I’ll bother with this upgrade again. Business Select really gets you four things, some of which aren’t really what they’re cracked up to be. Priority Boarding (A15 or lower):  I was A3 and A2 for both legs of my flight. However, since my flight originated in Sacramento before arriving in Phoenix, they allowed everyone who originated in Sacramento to board ahead of the A group, so even boarding as an A2 there were already close to 15 people on board. The best aisle seat I could find with overhead space for my carry–on was in Row 6. And in both cases, an unusually large person from boarding group C  sat in the middle seat, making it a very uncomfortable flight – even with priority boarding, it’s still luck of the draw who sits next to you I guess. A free drink –  at a whopping $5 value. I actually got two drink coupons since I had a layover, so make that a $10 value, although I didn’t get to use either of them since I was traveling on business…One way to improve this incentive would be to allow it to be unlimited drinks as opposed to just one like other airlines do for First Class, and include full bottles of water and other premium NA beverages for those that aren’t drinking (Vitamin Water, Gatorade, premium Iced Tea, etc.). An Extra Credit – this might be the only value–add that was worth it. Since my flight was over 750 miles, I got two credits instead of one. For the $35 fee, it would only take me 8 flights instead of 16. This works out to 8 flights x $35 = $280 extra to earn that extra round–trip. The math kind of works out, especially if you’re cashing in those drink coupons. But if any flights are under 750 miles, that extra credit is only a quarter of 1 credit (.25) extra so then the value gets thrown off considerably. Fly–by Security Lane – this can be a huge help sometimes, other times not so much. It probably depends on what airport you fly out of most frequently – PHX is usually pretty efficient. Even saving 5 minutes can be nice though.  Between the extra credit and the shorter security line, I’d say this is where the real value of the upgrade is at. In the end, I’d say that $35 is about the most I’d pay for this package, and even then, it would probably be awash in the long …

How to Sneak Booze Onto a Cruise Ship Trying to sneak some alcohol onto a cruise ship? I don’t blame you – drinks on cruise ships are very expensive! Sneaking liquor onto a cruise ship can be a bit tricky, especially for first–time cruisers or anyone that has never tried it before. The cruise lines all state what their alcohol policies are and warn you that if they find anything else, it will be confiscated. In reality, not everyone gets searched – my girlfriend and I walked onboard our last cruise without anyone bothering to look through our carry–ons, and we could have easily snuck more liquor on, but other people we spoke to weren’t so lucky and got a thorough searching and all of their prohibited alcohol taken away. Based on my personal experience, as well as the experience of other cruisers I have spoken to, here is my best advice for anyone thinking of sneaking extra booze onto a ship. 1.Sneak on booze you’re willing to lose_  the worst that will happen is that they take it and not give it back, so don’t bring your fanciest, most expensive type of liquor. Leave the bottles of 21-year-old scotch at home, and opt for cheaper liquor that won’t upset you terribly if it gets confiscated. 2.Check what you are allowed to bring on the cruise ship’s website. For instance with Carnival, you can bring two 750 ml bottles of wine, plus 12 non–alcoholic drinks 20 ounces in size or less. 3.Figure out what container or bottle you are going to funnel your liquor into. This is very important, as you don’t want to bring it in something super obvious, but you don’t want something that will spill or leak either. …
